About the Role

Optum House Calls is seeking Nurse Practitioners and Physician Assistants to conduct annual in-home health assessments for Medicare Advantage and other health plan members. This is a non-prescribing, field-based role focused on preventive care, health education, identifying care gaps, and partnering with primary care providers to improve patient outcomes.

Key Responsibilities
  • Conduct comprehensive in-home health assessments
  • Review medical history and medications
  • Perform physical exams, screenings, and point-of-care testing
  • Identify and document diagnoses for care management and treatment planning

    Required Qualifications
    • NP: ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
    • PA: NCCPA certification and active state licensure
    • Active, unrestricted license (or ability to obtain by start date)
    • Valid driver's license and reliable transportation
    • Ability to work independently in patients' homes and varied environments
    Preferred Qualifications
    • 1+ years of clinical experience in family practice, geriatrics, primary care, or home health
    • Experience with Medicare or geriatric populations
    • Proficiency with EMR systems and technology
    • Strong communication and patient education skills
    • Ability to transport equipment up to 30 pounds and navigate stairs

    Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
