Hi. We’re New Horizons In-Home Care. We care for older adults beginning to need some help around the house, seniors who want to continue living at home, individuals with disabilities, and medically fragile children. We’ve been providing care to the Oregon community for over 30 years.

Responsibilities
  • Coordinate and monitor services, including tracking of client activities in relation to the client’s care plan
  • Oversee the maintenance of comprehensive client files and other documentation of all client encounters
  • Perform home visits as needed
  • Receive and process telephone calls and electronic communications
  • Assist in development of home service plans
  • Ensure Oregon Administrative Rules are followed
Qualifications
  • High School Diploma or GED equivalent
  • Valid Oregon Driver License with valid auto insurance
  • 2+ years in care management preferred
  • Possess strong written, verbal, and presentation skills with diverse populations
  • Ability to multi‑task and stay calm in a fast‑paced environment while ensuring attention to detail
