Three Oaks Hospice (and its sister companies) is a growing hospice and palliative care organization seeking compassionate professionals for nursing roles.

Position Summary

Primary functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency, and assume responsibility for coordination of care.

Essential Duties
  • Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patient’s physical, functional, psychosocial, and environmental needs and ADLs, including risk for grief, cultural/spiritual, verbal/non-verbal considerations.
  • Implement the individualized POC and recommend revisions to the plan as necessary.
  • Consult with and educate the patient/family regarding disease process, self‑care techniques, end‑of‑life care, nutrition and dietary needs; provide training to other staff as needed.
  • Initiate appropriate preventive and rehabilitative nursing procedures.
  • Prepare clinical and progress notes that demonstrate progress toward established goals.
  • Coordinate all patient/family services and prioritize needs with members of the IDT.
  • Use a case‑management approach and refer to other services as needed.
  • Inform physician and other personnel of changes in the patient’s needs, outcomes of intervention, and evaluate patient/family response to care.
  • Determine scope and frequency of services needed based on acuity and patient/family needs.
  • Assess the caregiver’s ability to meet the patient’s immediate needs upon admission and throughout care.
  • Evaluate personal needs for support and use identified systems to meet those needs.
  • Communicate information using current processes and technology available to the organization.
  • Apply specific criteria for admission and re‑certification to hospice care to establish appropriate levels of care and determine patient eligibility.
  • Perform on‑call responsibilities and provide on‑call service to patients/families as assigned.
  • Other duties as assigned.
Qualifications
  • Graduate of an accredited associate or bachelor’s degree in nursing.
  • Current state license as an RN and current state driver’s license.
  • Two years’ experience as an RN in a clinical care setting; previous hospice or home health experience preferred.
  • Current CPR certification.
  • Proficient computer skills required;
    Microsoft Office suite experience preferred.
  • Strong interpersonal skills and ability to provide effective support to patients and families.
  • Demonstrated skill in nursing practice, ability to cope with family emotional stress, and tolerance of individual lifestyles.
  • Strong communication and organizational skills, with a high standard of ethics and compliance.
