Essential Functions Statement(s)
- Manage patient care and scheduling.
- Monitor, record and report symptoms and changes in patients' conditions.
- Maintain accurate, detailed reports and records.
- Process, validate, and analysis of OASIS.
- Create 485 from OASIS assessment.
- Modify patient treatment plans as indicated by physician orders and patients' responses and conditions.
- Consult and coordinate with health care team members to assess, plan, implement and evaluate patient care plans.
- Monitor all aspects of patient care, including POC being carried out as written and goals met.
- Instruct individuals, families and other groups on topics such as health education, disease prevention, and develop health improvement programs.
- Ensure the team is educated on the needs of individuals, families or communities, including assessment of individuals' home or work environments to identify potential health or safety problems.
- Inform physician of patient's condition as required.
- Observe nurses and visit patients to ensure proper nursing care.
- Professionally manage patient, staff, and doctor issues as needed.
- Act as a liaison between the doctor and staff.
- Direct and coordinate infection control programs, advising and consulting with specified personnel about necessary precautions.
- Recommend where appropriate drugs, medical devices or other forms of treatment, such as physical therapy, inhalation therapy, or related therapeutic procedures.
- Consult with institutions or associations regarding issues and concerns relevant to the practice and profession of nursing.
- Engage in training or research activities related to nursing.
- Record patients' medical information and vital signs.
- Order, interpret, and evaluate diagnostic tests to identify and assess patient's condition.
- Direct and supervise less skilled nursing or health care personnel.
- Consult and coordinate with health care team members and physician to assess, plan, implement and evaluate patient care plans.
- Monitor all aspects of patient care, including diet and physical activity.
- Instruct individuals, families and other groups on topics such as health education, disease prevention and childbirth, and develop health improvement programs.
- Assess the needs of individuals, families or communities, including assessment of individuals' home or work environments to identify potential health or safety problems.
- Perform other duties as assigned by management.

Skills & Abilities
Position qualifications:
1. Current, unrestricted Registered Nurse (RN) license in the state of practice
2. Minimum of one (1) year of clinical nursing experience required; home health experience preferred.
3. Current BLS/CPR certification
4. Ability to travel throughout the assigned service area to provide patient care in their homes. Ability to successfully pass a background check, drug screening, and any other pre-employment requirement
