About the Position

Requirements Candidate CVs need to have the month & date on their work experience w/all gaps in employment accounted for.
Chart closure within 72 hours
Schedule = 18 PPD, minimum to see 18 PPD
Monday-Friday 8-5
A decision is typically made within 7-14 days
Will accept MD/DO
Must have prior FQHC experience
Must have prior experience and be comfortable treating children with BH (anxiety) issues
Must have experience with Pediatric Chronic Care Management
Active DEA needed before the start
Credentialing takes 30-60 days (six weeks is standard for Locums who submit app./documents timely)
The following immunizations are required for credentialing (before the start date):
Flu Vaccine, TB Status/X-Ray Results (results must be within 90 days before the start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years),
HCO & Vendor agree to a 30-day notice for ending all assignments.
