Position Summary

The registered nurse is accountable for the delivery and supervision of safe, quality care that is individualized for their assigned patients and follows the nursing process in accordance with the Nurse Practice Act (assessment, planning, implementation, evaluation). They are responsible for the assessment, treatment, and care of patients in the populations and ages assigned. The registered nurse embraces professional development, accesses, analyzes, customizes, coordinates, and communicates each patient’s plan of care and activities collaboratively with other members of the health care team.

Families or significant others are included within the plan of care as appropriate. They direct, delegate, and manage the care delivered by colleagues and subordinates, performing all duties in a manner that respects and supports the family centered care model. Collegial and collaborative care reflects the philosophy of the Division of Nursing, the Mission and Core Values of the Sisters of Providence, and National Patient Safety Standards.

Position Specific Responsibilities
  • Possesses knowledge of developmental needs and is competent in assessment and treatment relative to the age of the patient population served.
  • Conducts and documents a comprehensive patient assessment upon admission.
  • Continually reassesses and applies appropriate advocacy and interventions as indicated.
  • Interprets a patient’s self-assessment and behavior, understanding cognitive, physical, emotional, psychosocial, and chronological maturation processes.
  • Actively involves patient and family in the collaborative plan of care.
  • Consistently demonstrates and promotes safe patient care practices, including hand washing, patient , communication, infection risk reduction, medication safety and reconciliation, falls prevention, and inclusion of patient/family concerns.
  • Collaborates with the patient, family, and members of the healthcare team to develop, document, implement, and monitor an individualized plan of care based on evidence-based best practice.
  • Provides, documents, and consults to ensure necessary patient and family education in support of the treatment plan.
  • Adapts to change and updates patient care practices within an evolving healthcare environment.
  • Collaborates with team members, the patient, the patient’s family, and others to develop a proactive discharge plan.
  • Performs procedures and treatments according to accepted department/unit protocols, guidelines, standards, and the Nurse Practice Act.
  • Legibly and electronically documents patient care activities and information accurately, concisely, and timely.
  • Familiar with input and retrieval of information within various forms of written and electronic documentation.
  • Provides safe, quality care efficiently and cost-effectively.
  • Supports and assists with orientation of new staff and students.
  • Acquires training and skills to perform preceptor and charge nurse functions when selected.
  • Demonstrates professional nursing practice advancement through continuing education, required training, certification, and participation in department and unit activities.
  • Ensures all competencies, certifications, safety information, and courses are completed as required.
  • Identifies concerns and engages colleagues and management to promote a safe patient care environment.
  • Engages in positive communication, feedback, and ongoing development of self and team.
  • Builds and maintains an environment that fosters open communication, patient and family-centered care, and healthy collegial relationships.
  • Communicates patient status using the S.B.A.R. method and quality/safety tools for handoff.
  • Uses the Providence Early Assessment Team (P.E.A.T.) when indicated.
  • Promotes and routinely offers comfort and pain management measures.
  • Documents assessment, treatment, and reassessment to meet patient goals.
  • Incorporates safety and infection control principles per policy, procedure, National Patient Safety Goals, and emergency response procedures.
  • Complies with hospital standards (JCAHO, OSHA, State and Federal requirements).
