Position Overview

Eureka Community Health & Wellness is seeking an experienced Physician Assistant or Nurse Practitioner for locum tenens coverage beginning May 2026. This role is ideal for an advanced practice provider with a strong background in FQHC settings who is comfortable managing pediatric patients with behavioral health needs, chronic conditions, and some chronic pain management. The position offers a stable weekday schedule with no call requirements.

Clinical Responsibilities

• Outpatient pediatric primary care
• Management of pediatric chronic care conditions
• Evaluation and treatment of children with behavioral health concerns, including anxiety
• Participation in Pediatric Chronic Care Management programs
• Management of select chronic pain conditions
• Coordination of care within an integrated FQHC model
• Timely completion of documentation with chart closure required within 72 hours

Schedule & Workload

• Monday–Friday, 8:00 AM – 5:00 PM
• Minimum expectation of 18 patients per day
• No evenings, weekends, or call

Qualifications

• Physician Assistant or Nurse Practitioner
• Minimum of 5 years of experience in a similar outpatient or FQHC setting
• Prior FQHC experience required
• Demonstrated experience treating pediatric patients with behavioral health needs
• Experience with Pediatric Chronic Care Management
• Active California license
• Active DEA required prior to start date
• PA Requirements:
  California Controlled Substance Education Course Certificate
• NP Requirements:
  Furnishing License with Schedules II

Credentialing

• Estimated credentialing timeframe: 30–60 days
