About the Position
The RN Case Manager coordinates comprehensive patient care to ensure quality outcomes and efficient care transitions across the continuum of care. This role focuses on assessment, discharge planning, utilization review, and patient advocacy. The RN collaborates with physicians, social workers, insurers, and community agencies to optimize patient safety and satisfaction.

Requirements
Requirements:

Active RN License  – State specific or Compact (as applicable)

Experience:

1–2 years of acute care experience (Case Management preferred)

Certifications:

BLS required (ACM or CCM preferred)

Knowledge of discharge planning, community resources, and utilization review processes

Strong communication, critical thinking, and organizational skills

Ability to work independently in a fast-paced setting
