Connected Health Care is partnering with a respected Skilled Nursing Facility in Eureka, California, to hire an experienced Local Travel Licensed Vocational Nurse (LVN) for a 13‑week contract assignment. • Provide direct patient care to residents in a Skilled Nursing Facility
• Administer medications and treatments as prescribed by physicians
• Monitor and document patient conditions and progress
• Assist with assessments and development of individualized care plans
• Collaborate with Registered Nurses, physicians, therapists, and support staff
• Ensure compliance with facility policies, state regulations, and quality standards
• Respond to changes in patient conditions and communicate updates to the healthcare team
• Educate residents and family members regarding care plans and treatments
• Maintain accurate and timely patient records
• Support a positive, compassionate, and patient‑centered environment

• Current California Licensed Vocational Nurse (LVN) license in good standing
• Graduate of an accredited Practical Nursing or Vocational Nursing program
• Current BLS (Basic Life Support) certification required
• Minimum of 2 years of experience in a skilled nursing facility or acute care setting
• Strong clinical assessment and communication skills
• Ability to work independently and as part of a collaborative healthcare team
