Connected Health Care is partnering with a respected Skilled Nursing Facility in Eureka, California, to hire an experienced Local Travel Licensed Vocational Nurse (LVN) for a 13‑week contract assignment. Job Description
- Provide direct patient care to residents in a Skilled Nursing Facility
- Administer medications and treatments as prescribed by physicians
- Monitor and document patient conditions and progress
- Assist with assessments and development of individualized care plans
- Collaborate with Registered Nurses, physicians, therapists, and support staff
- Ensure compliance with facility policies, state regulations, and quality standards
- Respond to changes in patient conditions and communicate updates to the healthcare team
- Educate residents and family members regarding care plans and treatments
- Maintain accurate and timely patient records
- Support a positive, compassionate, and patient‑centered environment
- Current California Licensed Vocational Nurse (LVN) license in good standing
- Graduate of an accredited Practical Nursing or Vocational Nursing program
- Current BLS (Basic Life Support) certification required
- Minimum of 2 years of experience in a skilled nursing facility or acute care setting
- Strong clinical assessment and communication skills
- Ability to work independently and as part of a collaborative healthcare team
