Care Manager RN – St. Joseph Hospital, Eureka, CA
This is a full-time 8‑hour day shift position located in Eureka, CA.
The RN Care Manager is responsible for carrying out effective case management functions for identified patients by planning, coordinating, negotiating, procuring, and managing the care and needs of complex patients and their families throughout the continuum of care. The role involves daily communication with medical staff, hospital personnel, and outside resources to ensure the appropriate utilization of resources in accordance with clinical and financial standards.
The RN monitors the medical record for variances to clinical and financial standards and intervenes to achieve optimal outcomes for the patient and the hospital.
- Associate's Degree in Nursing or a nursing diploma upon hire.
- California Registered Nurse License upon hire.
- Minimum of 2 years of acute care experience in a Medical Surgical or Inpatient setting.
- Minimum of 1 year of experience in care management or utilization review in any setting, or successful completion of the TIPS program or Case Management Orientation Program upon hire.
- Bachelor's Degree in Nursing or higher within 3 years of hire.
- Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification.
