Position Overview

The Palliative Care RN (Case Manager) is an integral member of the interdisciplinary Palliative Care team, providing compassionate, patient-centered nursing care to individuals living with chronic, serious, and/or complex medical conditions. The Case Manager focuses on symptom management, care coordination, patient and caregiver education, and advance care planning to improve quality of life and support patients across the continuum of care.

Essential Duties and Responsibilities
  • Provides comprehensive nursing services to patients enrolled in the Home-Based Palliative Care Program, including ongoing assessment, intervention, evaluation, and follow-up care.
  • Manages an assigned caseload in accordance with established standards, productivity expectations, and regulatory requirements.
  • Develops, implements, and regularly updates individualized plans of care in collaboration with the patient, caregivers, physician(s), and interdisciplinary team members.
  • Assesses and manages pain and other distressing symptoms using evidence-based palliative care practices.
  • Educates patients and caregivers regarding disease progression, symptom management, medications, goals of care, and available resources.
  • Coordinates care with the patient’s primary care provider, specialists, community resources, and other healthcare partners to ensure continuity and alignment of care.
  • Serves as a clinical liaison between patients, families, providers, and the interdisciplinary team.
  • Participates actively in interdisciplinary team meetings, case conferences, and care coordination discussions.
  • Triage calls from enrolled patients and families, assesses the needs of the patient and family, and coordinates needed care over the phone.
Minimum Qualifications
  • Graduate of an accredited nursing program.
  • Current California Registered Nurse license.
  • Minimum of one (1) year of experience as a professional nurse within the previous three (3) years or equivalent experience in a BSN program.
  • Valid drivers license, proof of acceptable automobile insurance coverage, and reliable transportation.
  • Reliable mobile phone with plan that provides good local coverage.
  • Basic computer skills.
Preferred Qualifications
  • Experience in hospice or home health.
  • Experience working effectively with an interdisciplinary team.
  • CHPN certification.
