Registered Nurse

The Registered Nurse provides general medical care and treatment to patients under the direction of medical providers.

* $10,000 Sign-On Bonus Available
* (Half immediately upon hire; half after one year of employment)

Main clinic will be Monfort Family Clinic - but must be able to "float" between other clinics.

Position Summary:

With a Quality, Customer First, and Compassionate approach, The Registered Nurse will:

* Provides disease case management for complex patients with chronic conditions and patients undergoing diagnosis and treatment for TB Infection (LTBI).

* Support and back up the Obstetrics RN occasionally by providing high-quality care to expectant mothers. This role involves stepping in as needed to assist with patient assessments, non-stress testing, case management, and ensuring a safe and nurturing environment for patients.

* Conducts medication reconciliation for annual wellness visits for patients, focusing on preventive care and health promotion.

* Monitors patients according to guidelines set by CDC, CCGC, State and Sunrise Community Health.

* Assists clinicians in prepping appropriate patients with history, treatments, exams, diagnostic procedures, and medication administration as required by an RN. Monitors patients for reactions or side effects from administered medications.

* Assists in ordering and maintaining medications assigned.

* Maintains complete and accurate patient records. Records patient medical information and vital signs.

* Advises patients on health maintenance and preventative care.

* Monitors, records, and reports symptoms or changes in patient's conditions.

* Consults and coordinates with healthcare team members to assess, plan, implement, or evaluate patient care plans. Modifies treatment plans where indicated.

* Supports Medical Assistants and medical staff with nursing triage questions.

* Performs case management review as assigned. (DM, OB, Cardiovascular Disease, Asthma, LTBI, etc.).

* Works closely with Director of Nursing, Chief Clinical Officer, Medical Directors, and site management to ensure efficient and consistent compliance with clinical and operational policies and procedures.

* Ensures patient needs are being appropriately addressed through scheduling and triage.

* Supports team to meet patient needs.

* Participates in collaborative activities as designated by CCO and CPSO.

* Follows and initiates policy for standing orders.

* Provides quarterly training to staff on current health care issues or medical procedures.

* Team Based Care - Clinical Care Collaboration - Works in collaboration and continuous partnership with providers, clinical staff, and community resources in a team approach to promote access to appropriate care in the goal of enhancing patient health.

* Pre-Visit Team Based Planning - Reviews daily schedule and chart by care team, ensures documented instructions are written on the schedule, typed into the EHR appointment note, or in the EHR as a flag or clinical list update.

* Accepts other clinical or administrative nursing assignments from CCO or DON.

Minimum Qualifications:

* Associate's Degree in Nursing (ADN)

* Bachelor of Science Degree in Nursing (BSN)

* Certified & Licensed as a Registered Nurse in the State of Colorado,

* 2-4 years related experience and/or training, or the equivalent combination of education and experience.

* Bilingual English/Spanish Preferred
