Physical Therapist - Evanston Full time Days $5,000 sign on bonus
Salary range: $41.00 - $53.30 hourly. Placement within the salary range is dependent on relevant work experience and internal equity. For positions represented by a labor union, placement is guided by the collective bargaining agreement. The Physical Therapist reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and regulatory and accreditation standards.
- A licensed physical therapist who complies with the Code of Ethics/Standards of Practice as outlined by their professional organization and the State of Illinois.
- The position includes direct patient care, teaching, participation on appropriate committees, and departmental process improvement.
- Serves as a role model for all therapists and clinical interns in professional, clinical, communication, education and leadership activities.

- Presents a friendly, approachable, professional demeanor and appearance.
- Provides accurate information and timely updates to patients and customers, addressing questions promptly or directing them to the appropriate resources.

- Maintains primary responsibility over patient plan of care through timely and consistent collaboration with treating PT/PTA staff.
- Demonstrates teamwork by helping co‑workers within and across departments; communicates effectively, respects diverse opinions, and acknowledges contributions of others.
- Uses organizational and unit/department resources efficiently.
- Manages work schedule efficiently, completing tasks and assignments on time.
- Contributes to opportunities and processes for continuous improvement.
- Participates in efforts to reduce costs, streamline work processes, and improve services.
- Demonstrates creative problem‑solving and adaptability when adjusting treatment for special circumstances.
- Demonstrates essential skill competencies in selecting appropriate evaluation and use of treatment modalities, equipment and protocols; performs re‑evaluations per guidelines and adjusts the treatment plan accordingly.
- Assumes responsibility for patient management from initial contact to discharge, ensuring safety and treatment outcome; maintains primary responsibility over patient plans of care through collaboration with PT/PTA staff.
- Adheres to department policy regarding evaluations, progress notes, and discharge summaries; all notes are documented in the electronic medical record (EMR).
- Completes daily notes that reflect patient response to treatment, performance, and assessment of status; meets all documentation requirements.
- Provides educational materials and methods appropriate to the patient’s level of understanding and motivation.
- Participates in committees, department processes, and special projects in accordance with clinical expertise and performance expectations.
- Works to develop and maintain professional relationships with members of the healthcare team (e.g., physician, nurse, case manager) and participates in internal and external marketing per department expectations.
- Creates a culture of respectful communication by listening, soliciting team input, maintaining openness and honesty, and acknowledging contributions.
- Accepts constructive feedback and makes timely changes as evidenced by coaching forms, chart audits, and team feedback.
