Our Innovative Mobile Physical Therapy Team!

This is an exciting opportunity for a hybrid Physical Therapist on our Mobile Team, delivering outpatient physical therapy directly into patients’ homes. The role also includes the use of telehealth services, with potential in‑clinic support depending on patient needs and census.

Primary Coverage Area

Evanston Area.

Ideal Candidate

Qualifications Required:
  • Bachelor’s Degree in Physical Therapy.
  • Licensed Physical Therapist in the State of Illinois.
  • BLS from the American Heart Association upon hire.
Preferred:
  • Masters or Doctoral Degree in Physical Therapy.
Responsibilities
  • Demonstrate Northwestern Medicine values of teamwork, patient first, integrity, and excellence.

  • Provide accurate information and timely updates to patients and customers; address questions promptly.
  • Use service recovery skills to solve problems or service breakdowns.
  • Maintain primary responsibility over patient plan of care, collaborating with treating PT/PTA as needed.
  • Demonstrate teamwork by helping co‑workers within and across departments; communicate effectively and respect diverse opinions.
  • Use organizational and unit resources efficiently.
  • Manage work schedule efficiently, completing tasks and assignments on time.
  • Contribute to continuous improvement opportunities.
  • Participate in efforts to reduce costs, streamline work processes, and improve services.
  • Apply creative problem‑solving and adapt treatments to special circumstances.
  • Select appropriate evaluations and utilize treatment modalities, equipment, and protocols; perform re‑evaluations according to guidelines.
  • Assume full responsibility over patient management from initial contact to discharge, ensuring functional progress, safety, and outcomes.
  • Adhere to department policy regarding evaluations, progress notes, and discharge summaries; document all notes in the electronic medical record.
  • Complete daily notes reflecting patient response, performance, and assessment; meet documentation requirements.
  • Provide educational materials and methods suitable to patient understanding and motivation.
  • Participate in committees, department processes, and special projects aligned with clinical expertise.
  • Develop and maintain professional relationships with healthcare team members (physician, nurse, case manager).
  • Create a culture of respectful communication: listen, seek input, engage in open conversation, and acknowledge contributions.
  • Accept constructive feedback and make timely changes.
  • Complete non‑patient responsibilities within established timelines.
  • Contribute to educational efforts, including in‑services, journal clubs, new‑hire orientation, and PI.
  • Actively participate in student clinical or observation programs as a mentor or host.
  • Maintain productivity standards.
  • Be willing to flex schedule or assignment for patient care needs and meetings outside normal hours.
