Endeavor Health is seeking an RN Patient Care Navigator for the Stroke Program in Evanston Hospital. The role is 24 hours/week, with two 12-hour shifts (7:00 AM–7:00 PM).
You will coordinate care for stroke patients, support acute interventions, and work closely with neurologists, APPs, and the stroke team to meet center standards. Requirements include a Bachelor's in Nursing, Illinois RN license, CPR (BLS), ACLS (or to obtain within 3 months), and NIHSS certification.
