Job Details
- Must have current nursing license with Nursys verification within 30 days prior to start.
- Pharmacology 1 or 2 exam required prior to start and updated every 3 years thereafter with minimum 80% score.
- Ascension Medical Surgical Exam required prior to start and updated every 3 years with minimum 80% score.
- Ascension Color Vision test completed prior to start and updated every 3 years.
- Ascension Core Competencies including HIPAA, Joint Commission, NPSG, OSHA topics completed within a year of start and updated every 3 years with minimum 80% score.
- Ascension EKG Exam required prior to start with minimum 80% score.
- Ascension Onboarding Requirements Education Attestation required prior to start.
- Ascension Nursing Contract Worker Onboarding required prior to start.
- Documented proof of two MMR vaccines or positive IGG Titer required prior to start.
- Fit Test required prior to start and updated annually.
- Documentation of Hepatitis B vaccination series completion or declination required prior to start.
- Influenza Vaccine required prior to start during flu season (Oct 1 - Mar 31).
- Positive PPD or Negative Chest X-Ray and Annual TB Questionnaire required prior to start.
- Statement of good health or completed physical exam prior to first assignment, updated annually thereafter.
- Driver's License/State be current and valid.
- 2 Step PPD OR IGRA (Quantiferon/Tspot) required prior to start.
- Documented proof of Varicella immunity or vaccination required prior to start.
- Tdap vaccination valid within 10 years required.
- Ascension Profile (Resume) required for submission.
- At least two professional references required prior to first assignment.
- Ascension Criminal Background Check Results required prior to start and updated annually.
- Ascension EPLS required within 90 days of start and rerun if break in service >90 days.
- Ascension Skills Checklist clinical only, to be completed within one year of start and updated annually.
- 12 Panel Drug Screen required within 30 days prior to start and updated annually.
- Ascension Education Verification required within 90 days of start.
- Ascension Confirmation of Assignment required upon initial assignment and update upon extension.
- Third-party employment verification for past seven years required and updated annually.
