We’re seeking a compassionate, detail-oriented Licensed Practical Nurse (LPN/LVN) to join a mission-driven primary care clinic serving a frail and elderly population
. This is a clinic-based role where you’ll work closely with physicians, RNs, and an interdisciplinary care team to deliver coordinated, patient-centered care in a supportive environment.

🩺 What You’ll DoDeliver delegated nursing services within a clinic setting
, under the supervision of the Clinic Practice Manager
Collaborate with physicians, RNs, and the Interdisciplinary Team (IDT) to implement individualized care plans
Administer medications, vaccinations, and treatments and document care accurately
Perform blood draws within scope of practice, as designated
Monitor, observe, and report patient conditions and responses to treatment
Triage patients within scope to determine urgency and care priorities
Educate patients and caregivers on medications, treatments, and care instructions
Prepare exam rooms, equipment, and assist providers during exams and procedures
Support daily clinic operations including appointments, patient flow, and urgent situations
Respond to incoming phone calls and address or route concerns appropriately
Maintain exam room cleanliness, stock supplies, and monitor medication/supply expirations
Assist with care coordination including specialist scheduling, hospital admissions, and reports
Maintain accurate medical records and meet all charting and documentation standards
Follow all organizational policies, OSHA guidelines, and infection control protocols
Participate in quality improvement and performance enhancement initiatives
Engage in ongoing training, education, and professional development
Perform additional duties as needed to support clinic and patient care operations
✅ Required QualificationsGraduate of an accredited school of nursing
Active LPN/LVN license in good standing in the required state
BLS certification (or ability to obtain within 90 days of hire)
Reliable transportation, valid driver’s license, and state-required auto insurance
Medical clearance and up-to-date immunizations prior to patient contact
Ability to pass a comprehensive pre-employment screening (background, license, education, references, drug screen)
1+ year of LPN/LVN experience (strongly preferred)
IV Certification (preferred)
Experience working with a frail or elderly population
Strong communication, teamwork, and patient-education skills
Ability to thrive in a fast-paced, interdisciplinary clinic environment
(Training provided upon hire if certain experience is not present)
