LPN Telephone Triage; On Site

This is an ON SITE position (no remote work)

You’ll support the clinical team by serving as a key point of contact for patients, providing phone-based clinical triage, guidance, and care coordination in a busy outpatient clinic environment.

This role focuses on assessing patient concerns, prioritizing care needs, delivering education, and ensuring timely follow-up with providers.

You’ll collaborate closely with physicians, advanced practice providers, and clinical staff in a team-oriented setting where patient experience, clarity, and access to care are top priorities
