River Bend Nursing and Rehabilitation is seeking an experienced and clinically skilled Preventionist/Unit Manager to drive facility-wide patient safety. This highly specialized clinical role focuses on preventing healthcare-associated infections (HAIs), managing wound care and skin integrity protocols, and overseeing the regulatory assessments for long-term care and Skilled Nursing Patients.
Rate:
Up to $43/hour depending on experience
1. Infection Prevention & Control
- Surveillance: Tracking patient outcomes to identify, investigate, and report healthcare-associated infections (HAIs) using CDC criteria.
- Policy & Compliance: Developing and enforcing protocols for hand hygiene, isolation precautions, and equipment sterilization in line with CMS, OSHA, and CDC guidelines.
- Outbreak Management: Conducting epidemiological investigations and managing facility responses to active disease outbreaks.
- Employee Health: Coordinating immunization records, exposure tracking, and communicable disease clearances with Human Resources.
- Assessments: Conducting routine skin rounds and performing specialized assessments, measurements, and staging of acute and chronic wounds.
- Care Plans: Developing and updating individualized wound care treatment plans in collaboration with physicians and nursing staff.
- Prevention: Designing preventative skin care programs (e.g., pressure ulcer prevention, turning schedules) to minimize facility-acquired skin breakdowns.
- Regulatory Oversight: Managing the clinical documentation and reporting processes for regulatory assessments (often tied to the Minimum Data Set or MDS) to ensure compliant reimbursements.
- Staff
Education:
Designing and implementing training modules regarding infection control practices, dressing changes, and new clinical products. - Quality Assurance: chairing the Infection Control/Quality Committee, compiling data, and presenting quarterly reports to executive leadership.
- Licensure: Typically requires an active, unencumbered Registered Nurse (RN) license.
- Experience: Over 1 year of experience in long-term care, infection control, wound management, or MDS assessment.
- Certifications: Certifications like Certification in Infection Control and Epidemiology (CIC), Wound Care Certified (WCC), or Wound, Ostomy, and Continence Nursing (WOCN) are highly preferred.
