A skilled nursing facility is seeking an experienced MDS Coordinator to take the lead in driving quality outcomes, maximizing reimbursement, and ensuring exceptional resident care.

• Strong knowledge of current MDS processes, care plans, and Medicare/Medicaid regulations
• Experience coordinating assessments and interdisciplinary communication
• Attention to detail with a passion for accuracy and compliance
• RN or LPN preferred
• Skilled nursing or long-term care experience highly valued
• Must be licensed nurse in IN or be able to gain nursing licensure in IN.

• Coordinate and oversee the MDS process from start to finish
• Ensure timely and accurate resident assessments
• Collaborate with nursing, therapy, and administrative teams
