GENERAL FUNCTION
The Licensed Practical Nurse provides nursing care under the supervision of an RN or physician, delivering treatments and monitoring patient health in the home setting.
DUTIES AND RESPONSIBILITIES- Provides skilled nursing care such as wound care, injections, catheter care, and medication administration.
- Monitors patient conditions and reports significant changes to the supervising RN or physician.
- Documents nursing care and interventions accurately and in a timely manner.
- Assists in implementing the patient’s care plan as directed by the RN.
- Educates patients and families about medications and care techniques within scope of practice.
- Maintains infection control standards and patient safety during all care activities.
- Participates in training and performance evaluations as required.
Must be a licensed Practical Nurse (LPN) in the state of Indiana. Requires understanding of nursing procedures, home health protocols, and documentation standards. Must be CPR certified.
PERSONAL RELATIONSHIPSWorks under the supervision of an RN. Maintains communication with patients, families, and interdisciplinary team members.
PHYSICAL DEMANDSRequires the ability to perform patient care activities such as lifting, turning, and transferring patients. Must be able to drive to patient homes.
WORK ENVIRONMENTWork is primarily in patient homes. May encounter varying home conditions and potential exposure to infectious diseases.
