About This Role

We are looking for a compassionate, skilled RN or LPN to join our assisted living team. You will provide direct nursing care and wellness oversight to our residents, support their independence and dignity, and serve as a clinical resource for our care team. What You'll Do
  • Conduct resident wellness assessments and monitor changes in condition
  • Administer medications and treatments per physician orders and ALF regulations
  • Develop and maintain individualized service plans in collaboration with the care team
  • Communicate changes in resident condition to physicians, families, and the care team promptly
  • Supervise and delegate tasks to medication aides and care staff on your shift
  • Perform skilled nursing tasks including wound care, treatments, and catheter care
  • Coordinate with physicians, home health, hospice, and outside providers as needed
  • Ensure compliance with state ALF regulations and community policies
  • Document care accurately and timely in the electronic health record
  • Participate in care planning, quality improvement, and resident satisfaction initiatives
Requirements
  • Current, unrestricted RN or LPN license in the state of practice
  • Graduate of an accredited nursing program
  • Current BLS/CPR certification
  • Assisted living, long-term care, or geriatric nursing experience preferred
  • Strong assessment, communication, and organizational skills
  • Genuine compassion for elderly residents and their families
  • Ability to work rotating shifts, weekends, and holidays
