The Heritage Center of Holiday Health Care is seeking a dedicated and reliable Full-Time MDS Nurse to join our team! This important role will focus on completing MDS assessments and participating in resident care conferences to ensure high-quality care for our residents. This is an on-site, in person role.

We welcome applications from candidates who may not have extensive experience in MDS but demonstrate a strong willingness to learn. Our team is dedicated to providing on-the-job training to help this person succeed.

Key Responsibilities:
  • Complete and submit MDS assessments for all nursing unit residents
  • Participate in and coordinate resident care conferences to ensure optimal care plans
  • Conduct unit rounds to assess resident function and team interaction
  • Ensure MDS is submitted on time in compliance with State and Federal guidelines
  • Collaborate with the health care team, including physicians, to create an interdisciplinary care plan for each resident
  • Provide education to residents, families, and staff regarding the care provided
  • Monitor the completion of ADL (Activities of Daily Living) assessments
  • Ensure all care plans are aligned with the MDS assessments and RAP guidelines
Qualifications:
  • Graduate of an accredited school of nursing
  • Active nursing license in the State of Indiana
  • Experience in long-term care nursing (preferred)
  • Working knowledge of the RAI manual, ICD-10 codes, case mix index, and strong assessment skills
  • Proficient with computers and MDS submission processes
  • Strong interpersonal and communication skills
