Responsibilities- Conduct patient assessments to evaluate health status
- Develop and implement treatment plans tailored to individual patient needs
- Prescribe medications in accordance with clinical guidelines
- Provide health education to patients to support disease prevention and management
- Manage patient follow-up to monitor progress and adjust care plans
- Maintain accurate and timely documentation of patient interactions and care
- Minimum of 3 years experience in family nursing
- Master's Degree in Nursing or related field
- Certification as a Family Nurse Practitioner (FNP)
- Proficient in care planning and coordination
