- Collect, compile, and disseminate necessary information on all referrals and admissions to Hillcrest staff when needed.
- Assist in developing initial treatment plan and goals upon admission to program.
- Orientate new residents to rules, procedures, and general residential programs and services.
- Coordinate visitation for residents with approved family members.
- Maintain communication with family members concerning resident appointments as required.
- Attend assessments with residents as needed.
- Oversee residents, including approval of visits and phone contacts.
- Maintain approved contact lists.
- Oversee the needs of the youth while ensuring follow-up and information is communicated as appropriate.
- Coordinate after-care services.
- Maintain communication with the referral agencies to assess community needs, trends in placement, and other relevant information.
- Assist referral source/agency in identifying other appropriate agencies or services if admission to Hillcrest is denied.
- Assist with coordination of treatment plan goals and resident’s needs with outside agencies and family members involved in the treatment of the resident.
- Maintain appropriate documentation.
- Provide family education.
- Weekly updates to placing agencies.
- Monthly program reports to placing agencies.
- Monitor resident’s grades on an ongoing basis and provide a printout monthly for the medical record if the Educational Coordinator is out.
- Represent Hillcrest in an appropriate manner while at school or court.
- Maintain required twenty hours per year of ongoing training.
- Transport residents as needed.
- Obtain/maintain CPR and First Aid certification.
- Facilitate other residential functions as assigned.
- Must hold a bachelor’s degree in social work or related human service field.
- Must have knowledge of relevant clinical issues and demonstrate clinical expertise for populations served.
- Must meet requirements as identified by State Licensing regulations.
