WHAT IS THIS POSITION RESPONSIBLE FOR?
  • Provides face-to-face community based case management and skills training services from a recovery/person centered philosophy and orientation.
  • Evaluates individuals to determine psychiatric stability, functioning with regard to daily living skills, personal strengths and limitations, and environmental needs utilizing the ANSA assessment instrument.
  • Provide community-based skills training and development services for adult client. Skills training and development will be determined based on individual/family’s identified needs.
  • Coordinates assessment results and translates the service needs into a Person-Centered Treatment Plan.
  • Upon completing Provisional period of employment, participate in the CSS weekend/holiday coverage rotation.
WHAT'S REQUIRED FOR THIS POSITION?
  • Bachelor's degree in Social Work, Psychology, Sociology, Criminal Justice, or a related Human Service field.
  • Knowledge of available community resources is preferred.
  • Previous work with the SMI population is highly preferred.
  • Demonstrated proficiency with interpersonal and writing skills.
  • Excellent communication, time management, and organizational skills.
  • Must have valid driver's license, auto insurance that meets the Indiana State minimum coverage requirement, and qualify for Company's vehicle liability insurance.
  • Candidates must pass required background checks including county/state checks, sex offender registry check, and drug screen.
