Mission-driven social service agency is seeking a hardworking, self-motivated Community Support Coordinator to provide support services to individuals with developmental disabilities.
Support may occur in a variety of settings and may include providing direct and indirect coordination, life skills instruction, coaching, emotional support, financial management, and home maintenance assistance.
* Flexible hours, 15-25 hours a week*
- Promote autonomy and self-determination.
- Actively support or promote socially valued roles within the individual's community.
- Assist with daily living tasks and life skills development.
- Provide indirect care services to those who work directly with individual served.
- Promote a positive public perception of both the individuals served and of oneself.
- Make scheduled and unscheduled visits to the homes of individuals receiving services.
- Participate in staff meetings and training.
- Implement, directly or through delegation, the goals and objectives of the service plan.
- Assist in accessing community resources.
- Monitor and address medical needs of individuals served.
- Communicate effectively with individuals, families, and other team members.
- Provide transportation to appointments and activities.
- Maintain accurate and current individual records and documentation.
- Attend team meetings and provide updates on individual progress.
- Experience working with people with disabilities.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Valid driver's license, proof of auto insurance, and vehicle registration required.
- Reliable transportation.
- Ability to communicate clearly in a variety of situations is essential.
- 18 years of age or older.
