Community Support Coordinator
- Evansville

Mission-driven social service agency is seeking a hardworking, self-motivated Community Support Coordinator to provide support services to individuals with developmental disabilities.

Support may occur in a variety of settings and may include providing direct and indirect coordination, life skills instruction, coaching, emotional support, financial management, and home maintenance assistance.


* Flexible hours, 15-25 hours a week*

Essential Duties and Responsibilities
  • Promote autonomy and self-determination.
  • Actively support or promote socially valued roles within the individual's community.
  • Assist with daily living tasks and life skills development.
  • Provide indirect care services to those who work directly with individual served.
  • Promote a positive public perception of both the individuals served and of oneself.
  • Make scheduled and unscheduled visits to the homes of individuals receiving services.
  • Participate in staff meetings and training.
  • Implement, directly or through delegation, the goals and objectives of the service plan.
  • Assist in accessing community resources.
  • Monitor and address medical needs of individuals served.
  • Communicate effectively with individuals, families, and other team members.
  • Provide transportation to appointments and activities.
  • Maintain accurate and current individual records and documentation.
  • Attend team meetings and provide updates on individual progress.
Required Qualifications
  • Experience working with people with disabilities.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Valid driver's license, proof of auto insurance, and vehicle registration required.
  • Reliable transportation.
  • Ability to communicate clearly in a variety of situations is essential.
  • 18 years of age or older.
