Psychiatry Caseworker
Working under the supervision of licensed social workers, responsible for coordinating discharges, scheduling aftercare appointments and referring patients to outpatient and inpatient programs. Develops awareness of community resources to meet the needs of patients based on treatment factors and population health. Schedules discharge transportation according to each patient's insurance coverage and discharge plan.
The inpatient psychiatry department at Everett Hospital provides a 22-bed adult unit and a 22-bed geriatric unit. Caseworkers are an integral part of the treatment team on both units, working closely with social workers and patients to make referrals and develop discharge plans. Both units are licensed by the Department of Mental Health and serve patients with acute and chronic psychiatric conditions across the age and diagnosis continuum.
The units are part of the broader Department of Psychiatry at CHA which is affiliated with Harvard Medical School and provides education and training for social workers, psychiatrists and psychologists. The psychiatry department provides comprehensive outpatient and inpatient care, including running two community behavioral health centers (CBHC), a large psychiatric emergency service, specialty outpatient care teams, multiple adult units at Cambridge Hospital and Everett Hospital and child and adolescent units at Somerville Hospital.
Caseworkers are employed throughout the psychiatry department and are crucial to the mission of delivering topnotch patient care.
Education/Training:
College degree or equivalent, relevant experience plus four years of experience in a relevant human service field. Although licensure is not required, training in social work to include a BSW degree and/or a LSW license is desirable.
Work Experience:
Experience working with relevant community populations, such as, people with chronic mental illness, cognitive disorders, substance abuse disorders and medical issues.
Both prior experience working in a hospital setting and bilingual skills is a plus.
Experience using electronic medical records is necessary.
Demonstrated experience working in multicultural and multidisciplinary settings required.
