- Use strong head‑to‑toe assessment skills to identify subtle changes in patient condition.
- Utilize experience with acute and post‑acute care (CHF, COPD, wound care, IV therapy, post‑surgical care).
- Triage and manage urgent clinical situations independently in the home setting.
- Perform medication reconciliation, teaching, and safety monitoring.
- Perform comprehensive wound assessment, care, and documentation.
- Ability to work independently while knowing when to elevate concerns.
- Comfortable making real‑time clinical decisions without immediate on‑site support.
- Strong problem‑solving skills in unpredictable home environments.
- Ability to manage a dynamic visit schedule and prioritize urgent visits.
- Efficient with point‑of‑care documentation and technology.
- Strong attention to detail while maintaining productivity expectations.
- Comfortable providing care in varied home environments.
- Flexible and adaptable to changing patient needs and schedules.
- Demonstrates cultural sensitivity and respect for patient autonomy.
- Current nursing license in good standing in Massachusetts.
- Graduation from an accredited nursing program (verifiable by transcript or diploma).
- At least one year of recent hands‑on medical experience within the past two years with adult or geriatric patients in acute care, emergency department, home health, hospice, ICU, med‑surg, sub‑acute or rehab units.
Weekly base pay: $58–63 per point, subject to negotiation. Full‑time commitment includes a $5,000 incentive. Weekly pay schedule. Flexible scheduling.
