Compensation: new hires start between $25.42 and $31.77 based on experience, qualifications, skills, competencies, internal equity, and market factors. While the full pay range is listed for this position for pay transparency, individual compensation is determined by various factors and may vary within the posted range.

WHAT YOU'LL DO

The Case Manager (Community Resource Navigator) assists in providing the most accessible and highest quality comprehensive customer service to individuals and families seeking services in Snohomish County. This position works to connect Carnegie Resource Center participants to available resources within the community, accurately collects client information and demographics, and assists service seekers in creating SMART goals to inspire healthy and productive living.

PRIMARY

DUTIES AND RESPONSIBILITIES
  • Provide needed information and resources to clients via phone, text, chat, email, and/or face‑to‑face conversations.
  • Engage with agency partners to support the needs of clients and community members.
  • Provide accurate information, referrals, and advocacy to all clients.
  • Deliver exceptional customer service and recommend improvements for quicker and more efficient service delivery.
  • Assist clients in accessing services and connecting with service providers in a professional, inclusive, and culturally sensitive manner.
  • Follow up with clients to capture outcomes and alternative options.
  • Support deadlines to complete assigned projects and new initiatives in a quality and timely manner.
  • Present informational updates to Carnegie Staff and Partners at regularly scheduled staff meetings.
  • Attend skill‑building training sessions as assigned.
  • Maintain acute awareness of Community Partner schedules to deliver accurate information to clients.
  • Maintain an up‑to‑date client tracking database and confidential client files.
  • Maintain and verify accuracy of the Community Resource Guide and all other pertinent information.
  • Handle multi‑line phone reception with a professional reception manner.
KNOWLEDGE, SKILLS, AND ABILITIES
  • Demonstrated experience establishing and maintaining effective working relationships with clients, staff, and community leaders.
  • Ability to remain calm, patient, and effective in tense situations.
  • Ability to express empathy when working and communicating with diverse client populations.
  • Experience working in a fast‑paced environment and shifting gears quickly to accommodate change.
  • Resourceful problem‑solving skills to handle client requests and obstacles.
  • Basic computer knowledge (e.g., email and MS Word) and office equipment operation; requires use of cell phone and laptop.
  • Professional communication with community members, government contractors, and facility clients.
QUALIFICATIONS FOR THE POSITION

(Any equivalent combination of knowledge, skills, abilities, education, and experience)

  • Associate’s Degree preferred; high school diploma or GED required.
  • 1 year of relevant experience in a human services‑related position.
  • Ability to quickly learn internal software applications.
  • Familiarity with Microsoft Office Suite products.
