Role OverviewThis role focuses on identifying the needs of individuals who are transitioning, or desire to transition, from institutional care to more independent living. You will provide support in helping them make their own choices and live in a fully accessible, integrated community or return to their own homes.
Responsibilities- Identify needs of individuals transitioning from institutional care to independent living.
- Provide support to help them make their own choices.
- Assist individuals in living in fully accessible, integrated communities or returning to their own homes.
- Bachelor’s degree in a social services related field or 2+ years of related work experience.
- Experience working with people with disabilities and/or older adults, preferably in an Independent Living Center or Aging and Adult services organization.
$19.00 - $22.00/hour depending on experience, competitive benefits, and flexible scheduling.
