SETTING: SNF/LTC - Conduit that connects individuals from the hospital to home, at the pace that is appropriate for them.
Initially, this partnership utilized 31 beds at Providence Pacific Campus. In November 2022 it expanded to a second floor, for a total of 62 beds dedicated to serve this population. START DATE: asap GUARANTEE: 36 - 40hrs sched ASSIGNMENT LENGTH: 13+ Weeks REQUESTED EXPERIENCE: Flexible, SNF/LTC exp preferred STAFF: Join team of therapists.
2 FT & 2 PRN EMR: Net Health(Optima) EMR CASELOAD / PATIENT SCHEDULING: 85% productivity expectation SCHEDULE: M-F, flexible if provider wants to work over weekend.
