Position Summary
The Home Health Aide is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Home Health Aide is responsible for carrying out assigned duties that promote patient comfort and support and activities of daily living.
Position Specific Responsibilities
* Provides personal care/hygiene to the home health patient, including but not limited to toileting and personal appearance.
* Assists the patient with transfers, exercise and/or ambulation.
* Turns and positions patients who are bed bound.
* Assists with Range of Motion (ROM) appropriate for the patient as directed by the RN or Therapist.
* Assists patients with meals and nutritional support.
* Changes simple non-sterile dressings or external bags per state and federal regulations.
* Changes bed linens and straightens patients' environment.
* Observes and reports changes in patient conditions and needs to the case manager.
* Participates in interdisciplinary team meetings (IDT) as appropriate to coordinate patient care, exchange information and problem solve, and receive staff support and in-service education.
* Participates in Company programs providing orientation, training, and education.
* Follows patient plan of care at all times.
* Participates in agency quality improvement as requested
* Ensures that all care is provided with respect for patient rights.
* Reports all grievances and complaints made by patients or families to the appropriate person(s).
* Adheres to established personnel policies.
* Reports all allegations of patient abuse and/or misappropriation of patients property as appropriate
* Follows standard precautions and infection control procedures.
* Reports all accidents and incidents observed.
* Identifies and responds appropriately to emergency situations.
* Observes safety needs of the patients.
* Attends in-service and meetings as required.
* Provides documentation of patients visit on the clinical record accurately, timely, and appropriately per agency policies.
* In conjunction with the nurse, develops treatment appropriate with the patients' disease progression.
* Addresses patients' care need with families and significant others as appropriate.
* Performs other duties as assigned.
Education and/or Experience
* High school diploma or GED highly preferred.
* One (1) to three (3) months related experience and/or training highly preferred.
Skills
* Mathematical
Skills:
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
* Language
Skills:
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
* Other
Skills and Abilities:
Ability to understand, read, write, and speak English. Articulates and embraces hospice philosophy. Working knowledge of home health aide principles, standards and applications. Knowledge of use of prosthetic devices, ambulatory aides, assistive and resistive devices, and training. Knowledge of medical terminology. Ability to work with all types and levels of patients. Strong organizational and time management skills. Must be able to work a flexible schedule to include nights and weekends.
Must be able to travel.
Certifications, Licenses, and Registrations
* Certified Nursing Assistant license required.
* Current CPR certification required.
* Preferred - 1 year Previous experience required as a nurse's aide, home health aide, orderly, or related experience in an acute, rehabilitative, or long term care facility, or home health/hospice organization.
Washington
* To provide home health aide services in Washington, an individual must be Certified Nursing Assistant (CNA) licensed by the Department of Health or have successfully completed a 75-hour home health aide training program approved by the Department of Health. Annual competency evaluation and ongoing supervision by a registered nurse is required. Driving may be necessary for this role. Caregivers must comply with state driving laws and requirements.
