Alpha Home Health and Hospice is a Medicare-certified, 5-Star rated agency seeking an experienced and caring Full‑time Bachelor’s Level Medical Social Worker (BSW) for our Hospice division in the greater Snohomish County area.

What does a Hospice Social Worker do?
  • Assess the psychosocial status of patients related to patient’s illness and progression
  • Develop and provide multiple community and governmental resources to support a patient in their home environment
  • Provide assessment in patient’s residence, carry out social evaluations, and plan interventions
  • Participate in the development of the individualized plan of care
  • Assist physician in understanding significant social and emotional factors
  • Participate in quality assessment and performance improvement team
  • Serve as liaison between patients and community agencies
  • Maintain and develop contracts with public and private agencies
  • Assist with bereavement coordination
Qualifications
  • A graduate of an accredited bachelor’s or master’s program in social work
  • Minimum of one year’s experience in health care is required
  • Experience with home health or hospice is strongly preferred
  • Possesses and maintains current BLS Certification
  • A licensed driver with an insured automobile
  • Requires weekend rotation:
    Saturday and Sunday 8‑5 or 9‑6.
  • A Tuesday through Saturday schedule is highly preferred but not required.
