Hospice Medical Social Worker | Master's

Alpha Home Health and Hospice is a Medicare-certified, 5-Star rated agency seeking an experienced and caring Full‑time Master's Level (MSW/LICSW) hospice social worker to serve the greater Snohomish County area.

• Assess psychosocial status related to patient illness and progression.
• Develop and provide community and governmental resources to support patients at home.
• Conduct assessments in patients’ residences, carry out social evaluations, and plan interventions.
• Participate in the development of individualized care plans.
• Assist physicians in understanding significant social and emotional factors.
• Participate in quality assessment and performance‑improvement teams.
• Serve as a liaison between patients and community agencies.
• Maintain and develop contracts with public and private agencies.
• Assist with bereavement coordination.

• Graduate of an accredited bachelor’s or master’s program in social work.
• Minimum of one year’s experience in health care (home health or hospice strongly preferred).
• Current BLS certification.
• Licensed driver with an insured automobile.

Weekend rotation required:
Saturday and Sunday 8:00–5:00 or 9:00–6:00;
Tuesday through Saturday schedule preferred, but not required.
