Sunday-Tuesday: 7:30p-7:30a
Essential Job Functions- Perform hands-on patient care at the place of assignment: in-home, facility, or adult family home.
- Provide quality care to patients/clients in an environment which promotes patient/client rights, dignity, freedom of choice, and individuality.
- Provide individualized attention and encourage residents to maintain or attain the highest practical physical, mental, and psychosocial well-being.
- Understand and ensure that individualized care plans are followed. Report any change of condition or observation of patient needs to the nurse supervisor.
- Assist with ADLs, including grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communication, or other needs as directed by care requirements.
- Promote skin integrity to prevent pressure ulcers, skin tears, and other damage by turning and repositioning immobile residents and applying moisturizers to fragile skin.
- Treat each patient/client’s personal property with respect.
- Perform delegated tasks assigned by the nurse, including vital signs, residents’ weights, and applying creams/ointments.
- Physically able to lift 50 lbs, using proper body mechanics or lifting devices for safety.
- Follow safety and infection control procedures including thorough hand washing, use of disposable gloves where indicated, and proper disposal of soiled materials.
- Maintain versatility, flexibility, and a willingness to work within changing priorities with a client-first attitude.
- Communicate and interact effectively and tactfully with residents, visitors, families, peers, and supervisors.
- Foster a culture of excellence and provide reliable, safe, high-quality patient care. Represent Journey Culture in the community.
- Perform other job-related duties as needed.
- High school graduate or G.E.D. certificate
- CNA or HCA license to provide home care in Washington State. Can complete training on-site
- Experience as a Home Care Aide (in homes or facilities), or other applicable experience preferred.
- Be listed in good standing with the Washington DSHS Background Check System
- Ability to read, write, speak, and understand English as needed for the job
- Have transportation to and from the job
- If providing transportation to clients, you must have a valid driver's license and insurance
- Population-specific experience a plus (training may be provided) for conditions such as dementia, Parkinson's, hospice/end-of-life care
- RN delegation certificate a plus; training may be completed through Journey Nursing Services.
