DescriptionSnohomish County Human Services Long-Term Care & Aging is seeking a qualified individual to support the Health Related Social Needs (HRSN) Program. HRSN services allow qualifying Medicaid enrollees to receive evidence-based, non-medical services to address unmet adverse social conditions that contribute to poor health. This program is being contracted to our Area Agency on Aging (AAA) through June 30, 2028, by the Home and Community Living Administration (HCLA) and the WA State Health Care Authority.
Since this is a new program, there will be an expectation of understanding that change will be inevitable in the following years.

- Desires to provide excellent case management to qualifying Medicaid enrollees of all ages.
- Has strong understanding of in-home assessments, care coordination, and public benefit programs.
- Desires to help individuals develop a service plan to increase their quality of life and delay the need for a higher level of care.

Role OverviewTo assist functionally impaired adults to obtain and effectively use necessary support services designed to achieve and maintain the maximum level of health and independence of which they are capable. The Case Manager maintains ongoing contact with clients to enable a prompt response to changes in the person's living environment.
Job Duties- A Master's degree in behavioral or health sciences; AND one (1) year paid on-the-job social service experience.
- A Bachelor's degree in behavioral or health sciences; AND two (2) years of paid on-the-job social service experience.
- A Bachelor's Degree in any field; AND four (4) years of paid on-the-job social service experience.
- Must pass job related tests.
Special Requirements: A valid Washington State Driver's License, unrestricted except as to vision, will be required prior to appointment.
