Occupational Therapist Home Health

Signature Healthcare at Home is seeking to add an Occupational Therapist for Snohomish County! This position would cover Snohomish County (Everett, Marysville, Lynnwood, Edmonds, Bothell, Lake Stevens, Mukilteo, Arlington) for home health occupational therapy services. It works Monday through Friday day shift, with self‑directed scheduling dependent on patient needs and territory. • Assume responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s).
• Provide patient, family, and/or caregiver education, counseling, and referrals to other home health services as appropriate.
• Evaluate the patient’s functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self‑care ability, work capacity, etc.).
• Evaluate the home environment for hazards or barriers to more independent living; identify equipment needs.
• Participate in the development of the total plan of care.
• Supervise Certified Occupational Therapy Assistants according to organization policy and state regulations.
• Participate in discharge planning for patients.

• Graduated from an OT program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) of the American Occupational Therapy Association (AOTA).
• Licensed and able to practice in Washington State.
• Eligible for or have taken the OT certification examination administered by the National Board for Certification in Occupational Therapy (NBCOT).
• Ability to drive and/or have access to transportation for agency‑related travel; possess a valid driver’s license, maintain automobile insurance coverage, and have access to an automobile.

• Clinicians with experience in Home Health or post‑acute care.
• Familiarity with Homecare Homebase EMR.

• Full‑time and Part‑time positions available Monday through Friday.
• Hourly $55 – $65 (dependent on experience) + mileage reimbursement.
• $5,000 sign‑on bonus for full‑time.
• Medical, dental and vision coverage beginning the 1st of the month after hire.
• Paid company holidays, 2+ weeks paid time off and 401(k) match after 90 days.
