This position is a split position between administration and caregiving responsibilities.

Benefits:

  • Sick Time: 0.5 paid sick time off every 20 hours worked.
  • Paid Time Off (PTO): PTO accrued at 0.038 per hour. (the equivalent of 80 hours annually based on a 40-hour work week). Benefits eligible to use after 6 months.
  • 401(k). Benefits eligible after 3 months of work
  • Employee Medical, Vision, and Dental Insurance. Benefits eligible after 60 days of work.
  • 12-days off for holiday office closures. On-call is still required.

    Monday
    - Friday .
  • This position is approximately 50/50 in the field and the office: 20-30 hours in the field per week.
  • On-Call Caregiving Requirement.

1), Martin Luther King Jr. Day (third Monday of January), Presidents Day (third Monday of February), Memorial Day (last Monday or May), Juneteenth (June 19th), Independence Day (July 4th), Labor Day (first Monday of September), Veteran's Day (Nov 11th), Thanksgiving Day (fourth Thursday in November), Native American Heritage Day (Friday immediately following fourth Thursday in November), Christmas Eve (December 24th), Christmas Day (December 25), The evening of New Year's Eve (Dec 31, 6 pm-midnight).

* Holidays are scheduled off with the exception of on-call/backup care requirements. *

Essential Job Functions:

  • Team Building:
    Support, lead, and train care givers in the field. This position is hybrid in the office and patients homes.
  • Case Management and oversight:
    Assist in patient admission process/ hands on patient care at place of assignment. Requires frequent visit to client homes; with the completion of plan of care updates, training for staff, compliance requirements, skill nursing visits, and shift coverage as needed.
  • Scheduling and staffing:
    Recognize staffing difficulties before they become problematic and request placement of job. Work with caregiving team and family to provide appropriate and adequate staffing. Investigates all unfilled shifts and takes appropriate actions.
  • Education:

    Backup trainer for offered courses.
  • Foster a high-performing culture of excellence that drives employee engagement and provides exceptionally reliable, safe, high-quality patient care.
  • Assist with administrative tasks, record keeping, and documentation.
  • Perform other job-related duties as needed.

Caregiving

Skills:

  • Perform hands-on patient care at the place of assignment:
    In-home, Facility, or Adult Family Home.
  • Provides quality care to patients/clients in an environment which promotes patient/ client rights, dignity, freedom of choice, and individuality.
  • Provide individualized attention and encourage residents to maintain or attain the highest practical physical, mental, and psychosocial well-being.
  • Understanding and ensuring that individualized care plans are followed. Report any change of condition or observation of patient need to nurse supervisor.
  • Assist with ADLs: including assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communicating, or other needs in keeping with the individuals' care requirements as directed.
