This per diem role offers part‑time opportunities for licensed social workers to provide comprehensive patient‑centric care management services for at‑risk patients in an acute care environment.

The Inpatient Licensed Clinical Social Worker (LCSW) Care Manager delivers professional, comprehensive care management services, including assessment and planning, coordination of care, discharge planning, documentation of interventions, regulatory compliance, and patient advocacy. The goal is to ensure continuity of care for vulnerable patients, identifying and addressing social, financial, cognitive/behavioral, or legal barriers to accessing necessary resources.

• Master’s Degree in Social Work from an accredited Council on Social Work Education program
• Washington Clinical Independent Social Worker license upon hire
• At least 2 years of social work experience in any healthcare setting
• 1 year experience in an acute care setting or successful completion of a TIPs program or Case Management Orientation Program

• Basic understanding of acute medical‑surgical healthcare terminology
• Prior case management experience

Min: $49.41 Max: $76.38

The Inpatient Master of Social Work (MSW) Care Manager provides comprehensive, patient‑centric care management services, encompassing assessment and planning, coordination of care, discharge planning, documentation of interventions, regulatory compliance, and advocacy. The goal is to ensure continuity of care for vulnerable patients, identifying needed resources to address social, financial, cognitive/behavioral, or legal barriers to care access.

• Master’s Degree in Social Work from an accredited CSWE program
• Washington Clinical Independent Social Worker Associate license upon hire
• At least 1 year of social work experience in any healthcare setting
• 1 year experience in an acute care setting or successful completion of a TIPs program or Case Management Orientation Program

• ACM/CCM certification
• Basic understanding of acute medical‑surgical healthcare terminology
• Prior case management experience

Care Manager MSW (Everett, WA), Salary Range:
Min: $46.96 Max: $72.60

In addition to the base pay range, this role may offer shift differentials, standby/on‑call compensation, overtime, premiums, extra shift incentives, and bonus opportunities.
