- DOE Full-time Monday to Friday, 8:30 AM to 5:00 PM

- At least 2 years of intake experience in a medical office, preferably home health/hospice
- Experience with Home Care Home Base is strongly preferred
- Demonstrated cooperation and excellent customer service skills
- Ability to multi-task and stay on task in a fast-paced office
- Excellent team-oriented, verbal, and written communication skills
- Perform patient intake, verify insurance, demographics, orders, and document necessary information
- Interface with contract, clinical, and office staff to foster collaboration and positive working relationships
- Serve as the primary responder to all inbound calls and inquiries
- Provide excellent customer service to patients, referral sources, and community contacts
- Maintain proper daily workflow and ensure compliance with all regulatory requirements
- Communicate effectively and professionally regarding coordination of care expectations
- Educate and enforce deadlines, schedule updates, and workflow requirements
- Coordinate initial patient assessments and follow-ups with clinical staff
- Ensure accurate data entry and record-keeping for all patient information
- Assist in the development and maintenance of intake procedures and protocols
