Everett Center in Seattle seeks an experienced MDS Coordinator to support resident-centered care in our Healthcare & Medical Services department. In this role, you will complete and manage MDS assessments, ensure regulatory compliance, collaborate with interdisciplinary teams, and support care planning to optimize clinical outcomes and reimbursement accuracy. You'll educate staff on MDS processes, maintain thorough documentation, and participate in quality improvement initiatives.

Responsibilities
- Coordinate and manage the full MDS/RAI assessment process for all residents in accordance with federal and state regulations
- Complete and oversee timely and accurate MDS 3.0 assessments, including scheduling, data collection, and submission
- Collaborate with an interdisciplinary team to develop, implement, and update individualized care plans based on assessment findings
- Review clinical documentation to ensure alignment with MDS coding and to support appropriate reimbursement and quality measures
- Monitor Medicare and Medicaid requirements, including PDPM or related payment models, to optimize compliant reimbursement
- Analyze MDS data to identify trends, quality indicators, and opportunities for performance and outcome improvement
- Educate and support nursing and allied health staff on MDS requirements, documentation standards, and regulatory updates
- Ensure adherence to CMS guidelines and state regulations, participating in survey preparation and follow-up as needed
- Communicate regularly with physicians, therapists, social services, and families to ensure comprehensive and person-centered resident care
- Participate in quality assurance and performance improvement (QAPI) initiatives related to resident assessment and outcomes
- Maintain confidentiality and data integrity within electronic health record (EHR) and related clinical information systems
- Support Everett Center's mission by promoting evidence-based practice, patient-centered care, and community wellness
