• Perform comprehensive physical therapy evaluations and develop individualized Plan of Care with measurable goals.
• Implement evidence-based treatments including therapeutic exercises, gait and balance training, mobility and functional training.
• Monitor patient progress, adjust treatment plans as needed, and document outcomes in medical records per policy.
• Collaborate with an interdisciplinary team to support discharge planning and optimize functional independence.
• Ensure patient and staff safety, follow facility protocols, and uphold infection control standards.

• Active Physical Therapist license in Washington State or eligibility to obtain licensure.
• Doctor of Physical Therapy (DPT) degree or equivalent.
• Experience in a skilled nursing facility or geriatric rehabilitation setting preferred.
• Ability to work in a long-term care/SNF environment and contribute to an interdisciplinary team.
• Strong documentation, communication, and time-management skills; ability to manage a patient caseload within assigned hours.
