* * Home Health RN Responsibilities:**
* ** Conduct
* * initial and ongoing patient assessments in the home setting
* ** Develop
* * and update individualized care plans in collaboration with the care team
* ** Administer
* * medications, treatments, and injections as prescribed
* ** Perform
* * wound care and dressing changes and other complex care
* ** Monitor
* * vital signs and symptoms, reporting changes to the physician
* ** Educate
* * patients and caregivers on disease processes, medications, and self-care
* ** Coordinate
* * care with physicians, therapists, and other healthcare professionals
* ** Document
* * all visits and care provided in the EMR
* ** Ensure
* * compliance with agency policies, Medicare/Medicaid, and other regulations
* ** Participate
* * in case conferences and interdisciplinary team meetings
* ** Provide
* * emotional support and promote patient independence
** Requirements:
*** Washington RN licensure
* Home Health/Hospice agency experience preferred, but not required
* Current BLS certification
* Valid driver’s license and insured vehicle
