** Hospice RN Case Manager

Key Responsibilities **
* ** Provide
* * skilled nursing care and symptom management to terminally ill patients.
* ** Develop
* * and manage individualized care plans in coordination with the hospice team.
* ** Educate
* * and support patients and families through end-of-life care.
* ** Administer
* * medications and treatments for comfort and quality of life.
* ** Coordinate
* * care with physicians, social workers, aides, and other team members.
* ** Respond
* * to urgent needs and provide crisis support when necessary.
* ** Maintain
* * accurate and timely documentation in the EMR.
* ** Manage
* * a caseload, scheduling and prioritizing visits based on patient needs.
* ** Participate
* * in interdisciplinary team meetings and plan-of-care updates.
* ** Ensure
* * compliance with hospice regulations and standards.
*
* Qualifications:

*** Active Washington State Registered Nurse (RN) license.
* Prior Home Health and/or Hospice experience is strongly preferred.
* Current BLS certification from the American Heart Association.
* Valid driver's license and insured, reliable vehicle.
