The Inpatient Registered Nurse (RN) Care Manager provides professional, comprehensive, and patient-centered care management services to at-risk patients in an acute care setting. This role focuses on assessment, planning, coordination, discharge management, and resource utilization to ensure safe, effective, and efficient patient care transitions.

• Conduct comprehensive patient assessments and develop individualized care plans
• Coordinate multidisciplinary care across providers and departments
• Manage resource utilization and support regulatory compliance
• Lead discharge planning and ensure safe care transitions
• Advocate for patients and families throughout the care continuum
• Document interventions accurately and in accordance with regulatory standards
• Collaborate with physicians, social workers, and interdisciplinary teams

• Associate's Degree in Nursing (ADN) upon hire
• Active Washington State Registered Nurse (RN) License upon hire
• Minimum 2 years of acute care experience in a Medical-Surgical or Inpatient setting

• Bachelor's Degree in Nursing (BSN) or higher (to be completed within 3 years of hire)
• National Certification in specialty area
• 1+ year experience in care management or utilization review OR successful completion of TIPS Program or Case Management Orientation Program
