Cowlitz Family Health Center is a Federally Qualified Health Center currently seeking a Nurse Practitioner to join our team. In this role, you will deliver primary care medical services in a professional, optimistic, consumer-friendly manner rooted in evidence-based practices. You’ll work as part of a collaborative care team alongside medical assistants, receptionists, referral and records experts, care coordination professionals, behavioral health specialists, and outreach & enrollment staff.
Your responsibilities include diagnosing medical conditions, prescribing medications, monitoring potential side effects, and providing primary care and preventive health treatments.

10‑hour shifts with possibility of transitioning to 8‑hour shifts in the future.

- Active licensure to practice in Washington without restriction, probation, or limiting condition.
- Board certified or board eligible in Family Practice, Internal Medicine, or Med/Peds.
- Valid unrestricted license/registration to prescribe drugs, medications, pharmaceuticals, and controlled substances.
- No disciplinary action by any professional or peer review organization, governmental agency, licensing body, or hospital medical staff.
- Knowledge of evidence‑based primary clinical care standards.
- Demonstrated ability to collaborate effectively in a team setting.
- Ability to maintain effective professional relationships with patients and care‑team members.
- Strong communication skills.
- Ability to engage patients in improving and maintaining their health.
- Experience working with underserved, transient populations.
- Proficient computer skills and experience with electronic health records.
- Two or more years of related experience providing primary care in an outpatient setting.
- Bilingual (English/Spanish).
