Registered Nurse - Home Health

SCHEDULE:

REQUIREMENTS:
- Current state license as a Registered Nurse - Required
- CHPN certification - Preferred
- Proof of current CPR certification - Required
- Current driver's license - Required
- Must have reliable transportation and insurance - Required
- 1-2 years experience as a Registered Nurse in a clinical care setting - Required
- 2+ years Home health experience - Required
- Homecare Homebase Software Experience - Required
- OASIS Experience - Required

ROLES & RESPONSIBILITIES:
- Assess home health and/or hospice patients in stable and emergent situations to identify their physical, psychosocial, and environmental needs.
- Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Educate patients and/or their families regarding care, diet, medications, home safety, etc., using appropriate teaching methods.
- Supervise and provide clinical direction to the Home Health Aide/State Tested Nursing Assistant/Licensed Vocational Nurse to ensure quality service delivery.
- Arrange referrals to appropriate agencies when necessary and monitor assigned cases for compliance with third-party payer requirements.
- Communicate effectively and serve as a liaison to patients/family, team members, administration, physicians, and other healthcare professionals.
