Clinical Liaison (RN) – Hospital Partnerships & Strategy

Location: Everett, WA (On‑site / Hospital‑Based)

Compensation: $100,000 – $105,000 Annual Salary

Job Type: Full‑Time, Exempt

Reports to: Director of Partner Operations & Strategy

In this role, you aren’t just reviewing charts—you are a brand ambassador and a strategic partner. You will bridge the gap between hospital leadership, medical staff, and patients to ensure a seamless transition to my Laurel’s high‑acuity home care model. A Typical Day in the Life
  • Daily Integration: Attend hospital “round huddles” to identify high‑risk patients who would benefit from acute care at home.
  • Program Performance: Collaborate with Client Success to support program performance aligned with the hospital's clinical quality, readmission reduction, and throughput objectives.
  • Patient Advocacy: Educate patients and caregivers on the my Laurel program, ensuring a smooth transition and clear expectations for home visits.
  • Patient Identification: Conduct census and chart reviews to identify patients who meet clinical criteria for the acute care at home program, supporting appropriate care transitions in coordination with hospital care teams.
  • Provider Engagement: Serve as the clinical expert, educating hospital staff and providers on program benefits and outcomes.
What You Bring to the Team
  • Licensure: Current and unrestricted RN License in Washington State (or Compact State).
  • Clinical Edge: Minimum of 5 years of clinical experience in an Inpatient or Emergency Department setting.
  • Project Success: A proven track record of launching or scaling new healthcare services or programs.
  • Navigation

    Skills:

    EHR proficiency and the ability to navigate complex care systems and hospital workflows.
  • Communication: Exceptional interpersonal skills with the ability to influence hospital leadership and medical staff.
  • Education: Bachelor’s degree in Nursing (BSN) or a related healthcare field is required.
