– Key Responsibilities
- Provide skilled nursing care and symptom management to terminally ill patients.
- Develop and manage individualized care plans in coordination with the hospice team.
- Educate and support patients and families through end‑of‑life care.
- Administer medications and treatments for comfort and quality of life.
- Coordinate care with physicians, social workers, aides, and other team members.
- Respond to urgent needs and provide crisis support when necessary.
- Maintain accurate and timely documentation in the EMR.
- Manage a caseload, scheduling and prioritizing visits based on patient needs.
- Participate in interdisciplinary team meetings and plan‑of‑care updates.
- Ensure compliance with hospice regulations and standards.
- Active Washington State Registered Nurse (RN) license.
- Prior Home Health and/or Hospice experience is strongly preferred.
- Current BLS certification from the American Heart Association.
- Valid driver’s license and insured, reliable vehicle.
